Crouse Hospital Advance Directives Initiative
Crouse Hospital is committed to assisting as many people in Central New York in completing a healthcare proxy and/or Advance Directives forms. These orders ensure that a patient’s wishes about treatment are known and followed if he or she cannot speak for themselves due to incapacitating illness or injury. In the hospital you can request an Ethics Consultation to discuss difficult decisions regarding healthcare. Anyone, including physicians, staff, patients and their families may request a consultation.

This nonprofit organization has distributed more than eleven million copies of the Five Wishes living will, advocates for human dignity and compassion during treatment and end of life care, and presents advance care planning workshops.

This organization advocates for pain management and palliative care and provides information and resources about health care proxies, living wills, and a range of end of life issues. It offers support, counseling and local referrals for patients and families.

Hospice Directory
This site includes a national directory of hospice providers and offers information about coordinated care for patients with life-limiting illnesses.

The Center to Advance Palliative Care
This organization provides healthcare professionals with the tools, training and assistance needed to sustain successful Palliative Medicine programs. CAPC emphasizes that Palliative Medicine can be used as a patient’s link between all their healthcare providers.
