Hospitalist Service

Crouse’s hospitalists appreciate the trust that referring physicians show us by asking us to participate in their patient’s care. Communication is key in maintaining these relationships and in transitioning the patient’s care back to the primary care physician upon discharge.

Crouse hospitalists also provide medical consultative services and assist in treating patients of specialty physicians – orthopedic surgeons, general surgeons and neurologists for example – upon the physician’s request.

We work closely with primary care physicians by:

• making them aware their patient has been admitted and why
• requesting that copies of the patient’s admission note and discharge summary be provided to them
• securing follow-up appointments with the primary care physician for their patient prior to discharge
• securing primary care physicians for those patients who have no physician
