Financial Assistance for Addiction Treatment Services

Crouse Health Addiction Treatment Services is a National Health Service Corp (NHSC) approved site.  As an approved site Addiction Treatment Services offers a sliding fee scale based solely on family size and income.  The sliding fee discount program is applicable to all individuals and families with annual incomes at or below 200% of the most recent Federal Poverty guidelines.  Eligibility for discounts is based on income and family size and no other factors.

Addiction Treatment Services will not discriminate in the provision of services to an individual:

• Because the individual is unable to pay for health care services;
• Because payment for those services would be made under Medicare, Medicaid or the Children’s Health Insurance Program;
• Based upon the individual’s race, color, sex, age, national origin, disability, religion, gender identity or sexual orientation.
