MyHealthChart
MyHealthChart is a feature for current and past Crouse Hospital patients that helps you take a more active role in your healthcare. After setting up your account with a username and password, you or anyone else you designate, may view certain medical information related to your stay at Crouse. Upon discharge from our facility, you are able to view portions of your medical record, such as test results, immunizations, care plans and discharge instructions.

NOTICE: Crouse Hospital’s MyHealthChart is only for current or any previous Emergency or Inpatient Crouse patients since March 1, 2014. If you have not been a patient with us, we will not be able to provision a MyHealthChart for you.
Your “My Health Chart” patient account was created at the time of your discharge. Click here to access your portal account. The process below provides access to your health and wellness data that will enable you and your care team to make informed decisions.
You will be prompted to supply a validation code; last name; and date of birth. Your validation code will be your first initial, last name, month (2 digits) year (4 digits) of your registration.
