Onondaga County Community Health Survey

What do you think about the health of our community? If you are 18 or older and live in Onondaga County, the County Health Department wants to hear from you!

Take this 15-minute survey to help us learn more. Your answers will be private and not shared with anyone.

This survey is part of a “Community Health Assessment.” A key part of this is hearing about the health issues that are important to you. The Onondaga County Health Department will use this information to work on ways to improve the health of people who live here.
