New Hire Appointment
Crouse Hospital needs initial paperwork, including information required by state and federal law, to be completed before you begin working. Please bring these documents with you to your New Hire Paperwork Appointment:
- Two (2) forms of ID (driver’s license, social security card, birth certificate or passport)
- Any relevant professional licensure, registrations and/or certifications
- All educational degrees/diplomas
- Your smile! We will be taking a photo for your employee ID badge at this appointment

Health Assessment
You will be scheduled for a pre-employment assessment with our Employee Health Office (EHO). This is required to ensure all our employees are able to successfully perform their jobs, as well as to provide the best possible care and service to our patients.
When you visit Employee Health for your pre-employment assessment, please bring the following:
- Documentation of Tuberculosis testing. Two-step Tuberculosis testing within 3 months or Quantiferon Gold blood test within 12 months.
- Measles/Mumps/Rubella (MMR). Proof of vaccination or documentation of immunity.
- Hepatitis B. Proof of vaccination or documentation of immunity.
- Varicella. Proof of vaccination or documentation of immunity.
- Tetanus. Proof of vaccination.
- Covid/Influenza. Proof of vaccination.
- Parental consent form (please note, if you’re under the age of 18, this form needs to be completed for employment)
During your assessment in the Employee Health Office, our staff will complete the following:
- Fit Testing for TB Respirator Mask: Performed for those with direct patient contact.
- Vaccination Review. A review of your vaccination records will be performed and the need for any vaccines or titers will be completed per policy.
- Physical Examination: Completed to determine ability to perform the requirements and functions of your new position.
All offers of employment are contingent on being cleared by the Employee Health Office based on the above listed criteria. This assessment is completely confidential. The Employee Health Office will not share specific information with the Human Resources department or your new department.
