Medical and dental forms should be submitted directly to Excellus BlueCrossBlueShield.

Dental forms for union employees should be submitted directly to Lifetime Benefit Solutions (LBS) or Service Employee Benefit Fund (SEBF).

Dental forms for non-union employees should be submitted directly to Lifetime Benefit Solutions (LBS).

Vision forms should be submitted directly to Davis Vision.
