Community Health Improvement Plan
Crouse Health is committed to enhancing the health and well-being of Onondaga County residents. In keeping with this mission, Crouse supports the community by investing staff time, effort and resources into improving community health.
Crouse Health staff provides community health and health professions education; creates and/or improves access to healthcare services for vulnerable or underserved populations; provides support through time and talents; promotes community building activities.
Crouse Health works with the Onondaga County Health Department, Syracuse hospitals and other health and public health organizations to identify our community’s most challenging health priorities.
After a year-long process, two priorities were chosen from the priorities detailed in the New York State Prevention Agenda, 2019 – 2024. The Prevention Agenda is New York’s health improvement plan and is the framework for local and regional health improvement implementation plans.
