Milestones & Memories

Making a commemorative gift to the Crouse Health Foundation is a special way to remember a loved one, friend, neighbor or colleague; to honor an important person in your life on their birthday, anniversary or other special occasion; to celebrate the birth of a baby; and to express appreciation for or give recognition to anyone you know.

Commemorative gifts not only show you care, but they help the Crouse community in a very meaningful way. All donors receive a thank you letter, and an acknowledgment is sent to the person (or person's family) on whose behalf you have made your gift (the amount of your gift will not be disclosed).

Donating by Check
If making a donation by check for your commemorative gift, please make it payable to Crouse Health Foundation and mail to 736 Irving Avenue, Syracuse, NY, 13210. Indicate the reason for the gift, such as in honor of or in memory of, and the individual's name. If you would like an acknowledgment sent to the recipient or their family, please provide the name and address.
